First visit

Reception procedures for first visits (No. 11 New Visits and Japan Mutual Aid Association of Public School Teachers Member Reception)

  • Persons undergoing consultation at this hospital for the first time without a letter of introduction and members of the Japan Mutual Aid Association of Public School Teachers are asked to enter required items on the consultation request form and, together with the health insurance card/medical certificate (senior citizens' certificate, elderly benefits recipient certificate, infant medical certificate, etc.), submit the form to the No. 11 New Visits Reception Desk. A number ticket will be issued.
  • Persons who have a letter of introduction, please go through procedures at the special-purpose reception desk next to No. 11.
  • If you are uncertain of the department for consultation, please make use of "The General Information" to the right inside the main entrance.

Please bring your health insurance card, medical certificate, and letter of introduction (Patient Referral Document).

Our hospital has been designated as a regional medical support hospital. A letter of introduction from your regular physician is required to receive consultation on an outpatient basis.
A health checkup can be received without a letter of introduction, but from April 2018, regional medical support hospitals with 400 or more beds are obligated to charge first-time visitors who do not have a letter of introduction (*1) 5,500 yen (consumption tax included) for special or specified medical care coverage(*2), in addition to the first visit fee. If at all possible, please bring a letter of introduction from a physician.

Persons who fall under the following are not required to pay the special or specified medical care coverage.
  • Persons who have a letter of introduction
  • Persons who were transported by ambulance
  • Persons who undergo medical examination
  • Persons eligible for medical assistance under the Public Assistance Act
*If your health insurance card cannot be verified, expenses will be handled by out-of-pocket payment.
*We are unable to accept copies or faxes.
(*1)The Health Insurance Act stipulates cases such as the following as first visits.
  • The person undergoes care at the hospital for the first time
  • The person has undergone consultation at the hospital in the past but the condition at that time has been cured
  • The patient voluntarily discontinues medical treatment, and undergoes consultation anew
A person who has not visited the hospital for at least a month since the previous consultation may be handled as a first visit, depending on the condition treated in the previous visit.
(*2)What is special or specified medical care coverage?

Japan's medical system requires efforts toward the promotion of cooperation and division of functions among hospitals and clinics in regions.
In order to carry out this mission, cooperate adequately with regional hospitals and clinics, and provide appropriate medical care to large numbers of patients, our hospital prioritizes treatment for patients who bring a letter of introduction (Patient Referral Document) from other medical institutions.
Accordingly, patients who do not bring a letter of introduction (Patient Referral Document) from another medical institution on the first visit will be charged 5,500 yen (consumption tax included) for special or specified medical care coverage at the time of the first visit.

After registration of your insurance information and creation of a patient card, please take a seat in front of the reception desk and wait for the number on your number ticket to be called.

After you are handed a reception slip and medical questionnaire, please go to the department to be consulted and talk to the receptionist.

The following persons are asked to go through procedures at the No. 11 New Visits Reception Desk.
  • Persons who have a patient card but wish to undergo consultation at a department for the first time
  • Persons who intend to visit multiple departments, including one visited for the first time

Returning visitors

Please undergo reception using the return visit machine.

Following the instructions on the screen, insert your patient card and select the content of the consultation.

Bring the reception slip (A4 size) printed from the return visit reception machine and your patient card. If blood collection is scheduled, go directly to the blood collection room on the 2nd floor.
Other persons are asked to wait in front of the consulting department.
Consultations are performed in the order in which procedures are handled at the return visit reception machine.

When your turn for consultation comes up, the number printed on your reception slip will be shown on the display in front of the department.
*If you intend to visit more than one department, do so in order.
*Please note that, when undergoing consultation at multiple departments, consultation may not be possible due to consultation hours.
*This hospital gives priority to patients who are members of the Japan Mutual Aid Association of Public School Teachers and patients who are referred by regional medical institutions.
*After April 2018, patients who continue to visit this hospital at their own discretion even after referral by this hospital to another hospital will be asked charged 2,750 yen (consumption tax included) for special or specified medical care coverage when making a return visit.

Other persons

Persons who have a patient card but wish to undergo consultation at a department for the first time

Consultation request procedures are necessary at the No. 11 New Visits Reception Desk.
Enter all departments to be consulted that day on the consultation request form and, together with the health insurance card/medical certificate, etc., submit the form to the No. 11 New Visits Reception Desk.

Persons who intend to visit multiple departments, including one visited for the first time

Procedures for the clinical department to be consulted that day will all be performed at the No. 11 New Visits Reception Desk.
Enter all departments to be consulted that day on the consultation request form and, together with the health insurance card/medical certificate ( senior citizens' certificate, elderly benefits recipient certificate, infant medical certificate ) , etc., submit the form to the No. 11 New Visits Reception Desk.

Changing a consultation appointment

As a provisional measure to handle the sudden increase in appointment changes by telephone due to the COVID-19 pandemic, we will handle calls directly at the Appointment Center for one hour from 9:00 to 10:00 on weekdays. ( The Orthopedic, Ophthalmology, and Pediatric departments are excluded. )

After performing procedures at the No. 13 Payment Desk, please pay using the automated payment machine.
